• 60-minute new patient consultations and 30-minute follow-ups
• 24/7 direct access to Dr. Kermani via phone or text
• A dedicated Care Coordinator (available by phone, text, or email)
• Fast-tracked referrals to trusted specialists
• Post-hospital care coordination
• Thorough medical record review prior to the first visit
• Curated, off-site wellness events (in partnered communities only)
How is this plan paid for, and what does it cost?
This is an individual concierge membership—residents pay their own annual fee of $3,000. The cost includes all core services.
Optional add-ons:
• Full care coordination: $500/year
• House calls (up to 4/year in select communities): $1,000/year
What additional benefits are included in partnered communities?
Residents in partnered communities enjoy exclusive perks at no extra charge, including:
• Quarterly wellness outings that promote mind-body health through creative, social, and therapeutic experiences
• Guest invitations to select events
• Referral rewards for inviting others to join the program
These enhancements are available only in communities with an active partnership.
Can I still join if my community isn’t partnered?
Yes. Residents of non-partnered communities may enroll through our standard Individual Concierge Plan. While the medical benefits are the same, the lifestyle perks listed above are exclusive to partnered communities.
What if I need help after hours?
Members have 24/7 access to Dr. Kermani’s private cell phone and their Care Coordinator. Support is available any time—including evenings and weekends—for urgent concerns, medication needs, or follow-up questions.
What happens if I’m hospitalized or go to the ER?
Dr. Kermani remains involved throughout the hospital stay, coordinating with the hospitalist team and reviewing records before your follow-up visit. This ensures a seamless transition back to outpatient care.
Can family members be involved in my care?
Yes. With your permission, family members can be updated on care planning and scheduling. The Care Coordinator helps streamline communication—especially for out-of-town caregivers.
How do I know if my community qualifies?
