How does a concierge membership benefit our company? 

The Corporate Concierge Plan enhances employee well-being through faster access to care, longer physician visits, and 24/7 support—all without the delays and restrictions of traditional medical models. By minimizing absenteeism, reducing workplace disruptions, and supporting proactive health management, this plan serves as a powerful tool for recruitment, retention, and executive wellness. 

What are the benefits included in this plan? 

Each corporate concierge membership includes: 

• Unlimited office and telehealth visits with Dr. Kermani – no co-pays, no surprise bills
• Same- or next-day appointment availability
• 60-minute new patient consultations and 30-minute follow-ups
• 24/7 direct access to Dr. Kermani via phone or text
• A dedicated Care Coordinator for direct support and communication-via text, phone and email
• Post-hospital care coordination, including record retrieval and expedited follow-up
• Comprehensive review of all prior medical records before the first visit

This model supports high-performing professionals with responsive, relationship-based care— keeping them healthy, focused, and supported.

What is the cost of the Corporate Plan, and are there add-on options? 

The base membership is $3,000 per employee per year and includes all core services listed above.

Optional enhancements include:

• On-site workplace visits (up to 4 per year): $1,000 annually
• Full care coordination (scheduling labs, imaging, and specialists): $500 annually

Discount rates are available for groups of three or more employees. Customized proposals are available upon request.

What kind of access do employees have to the physician and Care Coordinator? 

Employees have direct access to both Dr. Kermani and the Care Coordinator via phone, text, and email, including 24/7 communication for urgent medical concerns. There are no call centers, waitlists, or automated systems. 

How quickly can employees be seen, and how long are the appointments? 

Appointments are typically available within 24 business hours. New patient consultations are scheduled for 60 minutes, with 30-minute follow-ups—providing time for thoughtful discussion and personalized care. 

Can the plan be offered to select employees or leadership? 

Yes. Many companies offer concierge care to executives, leadership teams, or key personnel as part of a high-touch wellness or retention strategy. Enrollment can be structured to support selective or tiered participation. 

How is employee health information managed? 

All health records and communications are protected under HIPAA. No health information is shared with the employer unless the employee provides written consent. 

What is required from the company to get started? 

Very little. Our team provides all onboarding materials and works directly with enrolled employees. Once participants are identified, we handle intake, medical record coordination, and care setup directly

What happens if an employee goes to the ER or is hospitalized? 

Dr. Kermani remains actively involved during hospitalization, coordinating with the hospitalist team and reviewing the patient’s medical history to ensure continuity. Hospital records are retrieved prior to follow-up visits to facilitate a smooth transition back to outpatient care. 

Will my concierge membership cover any costs of seeing specialists or getting lab/radiology completed?  

The concierge membership fee covers only visits rendered by our office.   

As a concierge patient, what happens if they have to go to the ER or be hospitalized?   

Dr. Kermani stays in close contact with the hospitalist. Because he is intimately familiar with the patient’s medical history and has access to his/her chart, he and the hospitalist will work together to provide the best continuity of care.   

After a hospital discharge, patients most often face the hurdle of getting the hospital records to their PCP- despite attempts from the provider’s office as well.

As a concierge practice, we are fully committed to obtaining all hospital records before the patient’s hospital/ER follow-up appointment with Dr. Kermani.
